At The Landing, we are proud of the productive working relationships that we have developed with referring professionals throughout Southern California and across the nation, and we welcome the opportunity to work with therapists, counselors, and others who have not previously referred a client to our program.
Traditionally, our most common referral sources have been primary care physicians, other doctors, therapists, hospitals, and other treatment centers. This list is by no means all inclusive, and we are happy to work with all professionals whose clients can benefit from the type and level of services that we provide.
Our dedication to superior service for referents begins with the referral process itself. Actually, process may be an overstatement, because all that a professional needs to do is make one simple phone call. Members of our admissions and intake staff are available 24/7 to take your call. Once you have provided our representative with the pertinent information, we can take it from there. We will work with your client to complete a pre-admissions assessment, develop a personalized treatment plan, and make all other arrangements that are necessary for him to be admitted into our program. If the client intends to use insurance to pay for his treatment, we will contact his provider to identify and access any benefits to which he may be entitled.
When a referred client enters treatment at The Landing, we make every effort to coordinate with the professional who made the referral. During the intake process, we request that the client authorize us to share information and coordinate care with all who have been involved in his care, including the professional who referred him to us. If the client approves, we will contact the referring professional directly to determine the type and frequency of updates that he or she would prefer to receive. In most cases, the client’s case manager provides weekly updates to the referring professional, with additional contact made when necessary.
The discharge planning process for each client begins the day he arrives at The Landing, so that by the time he is ready to transition home or to his next level of care, he and his case manager have identified all resources that he will need to support his ongoing recovery. When a client has been referred to us by a professional such as a therapist, counselor, or primary care physician, we will make every effort to ensure that his continuing care plan includes a referral back into that professional’s care.
